Waiver and Release of Liability
1. Acknowledgment of Risks
I understand that participation in recreational and competitive sports organized by Fulton Recreational Sports, LLC involves inherent risks of serious bodily injury, including but not limited to: strains, sprains, fractures, concussions, and other orthopedic injuries; cardiac events; collisions with players, equipment, or playing surfaces; weather conditions; participant or spectator misconduct; and equipment failure. I understand that Fulton Recreational Sports, LLC does not provide health insurance, and I am responsible for any medical expenses resulting from my participation.
2. Assumption of Risk
I knowingly and freely assume all risks, known and unknown, associated with participation in any events, games, or programs hosted by Fulton Recreational Sports, LLC. This includes all risks of injury, illness, death, or property damage, even if caused by the negligence of others.
3. Release of Liability
In consideration of being permitted to participate, I, for myself and on behalf of my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless: Fulton Recreational Sports, LLC, its owners, managers, employees, referees, volunteers, and affiliates, as well as any partner facilities, sponsors, or event hosts, from any and all liability, claims, demands, or causes of action arising out of or relating to any loss, damage, or injury, including death, that may be sustained while participating in any activity, whether caused by negligence or otherwise.
4. Medical Treatment Authorization
I authorize Fulton Recreational Sports, LLC staff or agents to secure emergency medical treatment for me in the event of an accident, injury, or illness during any event, including if I am unconscious or otherwise unable to make decisions for myself. This includes but is not limited to: transport to a hospital or urgent care facility; administration of first aid or CPR by staff or emergency responders; and medical evaluation, procedures, medications, or surgery as deemed necessary by licensed medical professionals. I acknowledge that I am solely responsible for any medical costs incurred and release Fulton Recreational Sports, LLC from responsibility for the outcomes of such treatment decisions.
5. Code of Conduct & Removal
I agree to follow all rules, guidelines, and codes of conduct set forth by Fulton Recreational Sports, LLC. I understand that fighting, abusive language, unsportsmanlike conduct, or reckless behavior may result in immediate removal from events without refund, and possible bans from future participation.
6. Media Release
I grant Fulton Recreational Sports, LLC permission to capture and use photos, videos, or likenesses of me during events for promotional, advertising, and social media purposes, without compensation or additional approval.
7. Universal Coverage & Duration
This waiver applies to all events and programs hosted by Fulton Recreational Sports, LLC, including leagues, tournaments, skills sessions, and informal games, at any location used by the business. It shall remain in effect for one calendar year from the signature date unless revoked in writing.
8. Binding Agreement
I confirm that I am at least 18 years old. I have read this waiver fully and understand that I am waiving legal rights, including the right to sue for injuries or damages that may result from participation.
